faxed to (616) 785-1468.  All purchase orders must have a valid MasterCard or Visa number with expiration date as well as a complete “ship to” address.  We cannot ship to post office boxes, FPO’s or APO’s.

Companies who wish to place an order without a credit card number must first complete the credit application and be approved for a line of credit (see credit application tab).

All orders must also include a valid email address for communication purposes.
